Request an Appointment
You may request an appointment through this website or you may call our office during business hours to set up your initial exam. This scheduled appointment is reserved especially for you. We request 24 hours notice if you need to cancel or change your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 24 hours in advance, you are subject to being charged an additional fee.
Our office hours are Monday thru Friday from 8:00 am – 5:00 pm.
